Disclosure & Authorization — Fair Credit Reporting Act (FCRA)
Success Nurse Agency Inc ("the Company") may obtain a "consumer report" and/or an "investigative consumer report" about you from a consumer reporting agency for employment purposes. These reports may include information about your character, general reputation, personal characteristics, mode of living, criminal history, motor vehicle records, education, previous employment, professional licenses and certifications, drug testing results, workers' compensation records (where legally permissible), and other information necessary to evaluate your suitability for placement as a healthcare professional.
Reports may be prepared and delivered by a third-party Consumer Reporting Agency (CRA). You have the right, upon written request made within a reasonable time, to request additional disclosures regarding the nature and scope of the investigation, a written summary of your rights under the FCRA, and a copy of any report obtained.
This authorization remains valid for the duration of your engagement with the Company and any renewal or reassignment, to the extent permitted by law. A photocopy or electronic copy of this authorization is as valid as the original.
